- Cancellation or rescheduling of appointments with less than 24 business hours is subject to a $100 cancellation fee.
- Online scheduling is not available for patients with Medicare or Medicare Advantage.

If you require an appointment outside these times, please contact us and we will do our best to accommodate you.
What to wear
Comfortable clothes that allow you to move and allow your physical therapist to access and observe the treatment area. A tank top if the area to be treated is your neck, upper back or shoulder. Shorts if the area to be treated is your lower back, hip, knee or ankle.
What to bring
- Your insurance card and ID if we are billing insurance.
- Claim number for Labor & Industries/Workers Comp or PIP (auto accidents).
- It is highly recommended that you contact your insurance provider prior to your first appointment so that you fully understand your physical therapy benefits.
- Copay due at time of service. We accept cash, check, all major credit cards, Venmo, Apple Pay and Android Pay.
- Complete the following registration forms and bring with you to your first appointment.
